Utilidad de la Periostina como biomarcador en el asma
Resumen
En la última década hemos sido testigos de un gran avance en el conocimiento sobre biomarcadores no invasivos del asma, sobretodo del asma eosinofílica. La esperanza es que esos biomarcadores puedan ayudar a identificar fenotipos de pacientes respondedores a terapias específicas, principalmente las nuevas terapias biológicas. La periostina se posicionó fuertemente como un biomarcador emergente del asma, asociado con la inflamación eosinofílica y el fenotipo molecular T2-alto, a raíz de la publicación en el año 2011 de un ensayo clínico con lebrikizumab (anti-IL-13), en el cual los mejores respondedores fueron los pacientes con niveles altos de periostina pretratatmiento. Igual que ha ocurrido con otros biomarcadores, la periostina no ha cumplido las expectativas iniciales muy altas, de sustituto de eosinofília bronquial sobretodo. La periostina carece además de especificidad para el asma, y parece que no discrimina con un punto de corte claro sujetos sanos de pacientes asmáticos.
Sin embargo, la periostina mantiene un cierto valor como biomarcador de inflamación T2 subsidiaria de tratamiento específico en pacientes con enfermedad obstructiva de la vía aérea (asma, EPOC, ACO). Además, la periostina se está posicionando en los últimos años como un biomarcador de remodelado de la vía aérea, medible por métodos no invasivos; también constituye un probable biomarcador de respuesta a diferentes anticuerpos monoclonales anti dianas T2. De momento, al igual que ha ocurrido con otros biomarcadores como la FeNO, somos los clínicos quienes debemos aprender a interpretar los valores de periostina en el contexto individual de las características de cada paciente.
